Qualifications

About the Role

Assertive Community Treatment Director 

About the Role

We are seeking a dedicated Social Worker or Mental Health Therapist to join our Assertive Community Treatment (ACT) program. This role combines clinical therapy, case management, and community-based outreach to provide intensive, individualized support for adults with persistent mental illness. In addition to direct client services, this position includes administrative responsibilities to ensure program compliance, documentation accuracy, and coordination across the ACT team.

Key Responsibilities

Clinical & Community-Based Services:

  • Serve as the primary therapist and treatment coordinator for a caseload of adults with serious and persistent mental illness (SPMI)
  • Provide individual, group, and/or family therapy as needed
  • Assist in monitoring prescribed psychotropic medication under psychiatrist supervision
  • Conduct psychosocial assessments and develop individualized treatment plans
  • Provide case management services, including crisis intervention and community linkage
  • Deliver in-home and mobile services to ACT clients to promote stability and recovery
  • Educate patients, families, and community stakeholders about available resources and services

Administrative Responsibilities:

  • Maintain accurate, timely documentation in compliance with state and agency standards
  • Track treatment progress and outcomes for reporting purposes
  • Assist with program audits and ensure compliance with ACT model requirements
  • Participate in staff meetings, case conferences, and interdisciplinary collaboration
  • Support training of new ACT staff and interns as needed
  • Contribute to scheduling and coordination of ACT service delivery to ensure full program coverage

About the Role

As a PMHNP at Harford Belair CMHC, you will play a critical role in the treatment and care of adults living with persistent mental illness. You will provide psychiatric evaluations, medication management, and ongoing re-evaluations while collaborating with a supportive multidisciplinary team.

Key Responsibilities

  • Direct the treatment of a caseload of individuals with persistent mental illness
